[ Summary ] With the increases in the number of aged and diabetic patients, various complications exist in many cases at the time of dialysis introduction. Also, severe complications, such as myocardial infarction, may be reasons for dialysis introduction.
Although avoidance of dialysis is recognized as the most important goal for pre-dialysis patient treatment, it is more important to prevent complications, which reduce ADL and QOL, and also are causes of death. The early referrals to nephrologists, have been reported to be effective for survival after the start of dialysis. Recently, various uremic toxins have attracted attention as factors related to cardiovascular complications. Therefore, before they accumulate in high concentrations, sufficient informed consent and selection of therapy for end-stage renal disease and introduction of therapy at suitable times is desirable.
