[ Summary ] Bone disease is one of the major complications seen in chronic dialysis patients. The serum levels of biologically active 1-84 PTH are clinically important for the treatment of renal osteodystrophy. While intact parathyroid hormone (i-PTH) assay react with PTH fragments, whole PTH assay can measure the only full-length biological active form 1-84 PTH. Recent data suggest that synthetic 7-84 PTH fragments inhibit the calcemic effect of 1-84 PTH, vitamin D-induced bone resorption and osteoclast generation. Whole PTH assay may be useful to evaluate adynamic bone disease in end stage renal failure.
