[ Summary ] Surgery for sleep apnea syndrome is devided into the retropalatal type and the retroliugulal type. One of these, Uvulopalatopharyngoplasty (UPPP) is the most popular retropalatal type. Findings on UPPP were published by Fujita in 1981, and widely repeated in domestic and foreign contries. Howevere, the rate of OSAS improvement has not been great. We cannot be satisfied with the current results. We considered that, the low rate of cure was due to the lack of the diagnosis of obstructive lesions in all patients. In the future, we must diagnose precise obstructive sites, and correctly adaptat these operations. If n-CPAP or dental conservative therapy is performed, nasal disease or phayngeal abnormalities must be checked, and otorhinal therapy may be necesary.
