[ Summary ] Colonic diverticulitis and its complications are briefly explained and a case experienced in a long-term hemodialysis patient is presented.
This disease is predominantly observed in the left colon in America and Europe, but in Japan it occurs mostly in the right colon. Patients with chronic renal failure due to polycystic kidney disease have a high incidence of diverticulosis and diverticulitis. The incidence of this disease in other kidney diseases, however, has not been clarified. It is desirable to diagnose the exisitence of the diverticle early, but it is not practical to perform barium-enema and colonofiberscopic examinations to all dialysis cases. These two examinations are necessary for patients who complain of recurrent lower abdominal pain, or who manifest bloody stools. If clinical symptoms and signs are not serious, the patients can be treated conservatively. On the other hand, an emergency operation is mandatory if patients show severe abdominal pain, pressure pain, rebound tenderness and leucocytosis or increased CRP, because abscess, perforation or peritonitis must be suspected.
