[ Summary ] Long-term home care of dialysis patients requires professional expertise and techniques as well as general support for daily life from family members and social services. For that purpose, care workers and medical personnel need to share common goals. We presume that the basis for continuous, integrated support is information exchange among all care providers. We are currently using a reference notebook, conferences between hospital staff and visiting nurses, and interviews with care givers and family members. In the real settings, we exchange information by using some of these strategies and are able to provide better support that meets the needs of dialysis patients.
