[ Summary ] In the nursing process, nursing diagnosis is widely performed. The North American Nursing Diagnosis Association (NANDA) proposed Taxonomy II as a new classification in the year 2000. For consistent nursing processes and appropriate nursing diagnosis, assessment tools are necessary to clarify comprehensive and individual problems in patients. For efficient and appropriate nursing diagnosis in hemodialysis patients, we developed a "Taxonomy II Database Form" and a "Taxonomy II Assessment Form" based on Taxonomy II. The advantages of the two types of assessment tools are a reduction in the patient information collection time, realization of effective case conferences in a shorter time, and improvement in the assessment ability of nurses. These assessment tools are expected to be applicable to future electronic processing as nursing support systems because of algorithmic nursing diagnosis and coding of nursing diagnoses.
