[ Summary ] SIRS induced by sepsis has a high mortality rate when treated with anecdotal antibiotic therapy. Recently, continuous hemofiltration, plasma exchange, and direct hemoperfusion with polymixin B-immobilized fiber (PMX) have been used in order to decrease inflammatory cytokines and endotoxins in the circulatory system. No conclusions have yet been reached as to the superiority of these modalities and their resultant therapeutic out comes. It is quite interesting to speculate that the amelioration of circulatory states when using PMX may be explained not only in terms of the removal of endotoxins, but may also be due to another mechanism, which may be discovered through further investigation.
