[ Summary ] Active vitamin D analogues are frequently used in patients with chronic renal failure, especially under maintenance renal replacement therapy for management of secondary hyperparathyroidism or occasionally in patients receiving anticonvulsants. They are used in order to prevent adverse effects, such as hypercalcemia, ectopic calcification, with or without acute inflammation, without aggravating hypoparathyroidism and/or adynamic bone disease. Corrected serum calcium, phosphorus, magnesium, plasma intact parathyroid hormone levels, bone specific alkaline phosphatase levels and 1-alfa-dihy droxyvitamin D concentrations in the plasma should be monitored at the start and also periodically during administration of analogues.
As a fundamental rule, the purpose of administration should be reconfirmed, resulting in successful use of active vitamin D analogues, especially in dialysis patients.
