[ Summary ] In clinical practice, episode of hypoglycemia were sometimes observed in connection with progressive renal failure on diabetic nephropathy. The need of insulin decreases after the institution of maintenance dialysis, because of the prolongation of insulin half life, and reduced food intake, due to anorexia related renal failure.
Oral glycemic control drugs are contrain dicated, because of the retention of drugs. On renal failure stage, the modification from oral glycemic drugs to insulin therapy is necessary. Dysfunction in glucose and lipid metabolism was observed in CAPD patients, compared to HD cases.
Efforts to achieve good glysemic control are necessary, because of the observation that glycemia forcasts survaival of diabetic patients on CAPD and HD.
