[ Summary ] To prevent the incidence and developmentof diabetic nephropathy, glycemic and blood pressure control, along with restricted protein (low protein) diets were essential. Additional treatment for hyperlipidemia or atheroscle rosis and smoking cessation were also important. From results gathered by megatrials over a several year period, important evidence was obtained concerning diabetic nephropathy. Some concrete target levels for glycemic and blood pressure control were shown. The best choice of antihypertensive agents was recommended. Many investigations reported beneficial effects from restricted protein diets for treatment of diabetic nephropathy. However, evidenced-based medicine remained in conclusive. Moreover, there was great deal of undetermined evidence and further study should be performed. Strategies for diabetic nephropathy should be considered, to combine clinical experience with scientific evidence.
