[ Summary ] One of the most important factors in the success of renal transplantation is the quality of the donor organ. The kidney should be fresh without any abnormalities in its functions. Moreover, the donor's HLA is preferably better matched with a recipient who does not possess any anti-HLA antibodies to the donor.
The current status of HLA Testing for the donor and recipient in Japan is described. The assays include HLA typing, crossmatch and anti-HLA antibody screening, Also the Recipient Selection Standard of the Ministry of Health and Welfare is introduced for either brain death or non-heart-beating donors.
A situation similar to the protocol above is outlined, namely the UNOS Kidney Allocation Policy as well as the current status of cadaveric renal transplants which are performed in much larger numbers in the USA. There is a significant difference between the two countries.
