[ Summary ] We reported on the choice of blood purification methods for routinely encountered post operative renal failure patients. The choice of method depends on the cause, the hemodynamics, purpose of method (solute clearance, water clearance etc.). Generally, if the patient is hemodynamically stable, intermittent hemodialysis is the method of choice. If the patient is hemodynamically unstable, a continuous blood purification method is chosen and many times a combination of hemodialysis and hemofiltration is performed. Many cases involve multiple organ failures (MOF) in addition to the kidneys. Because of this, a continuous blood purification method such as CVVHD is chosen. Life support is exercised through overall care, including multiply combined therapies, such as respirational and nutritional management. Energetic, intensive therapy is vitally important.
