[ Summary ] A major contributing factor to renal anemia is the failure of an adequate compensatory rise in erythropoietin(EPO) levels in response to a decrease in the concentration of hemoglobin. It is associated with impaired oxygen sensing and EPO production. Replacement therapy with recombinant human EPO has become the most rational therapy. Renal osteodystrophy is a multifactorial disorder, related to bone remodeling. Calcitriol deficiency as a major contributor to renal osteodystrophy, has led to the development of treatment regimens that can maintain normal serum calcium and phosphate concentrations, reduce parathyroid hormone secretion, and correct a calcitriol deficiency.
