[ Summary ] Hyperhomocysteinemia has been shown to constitute an independent risk factor for atherosclerosis and arteriosclerosis in patients with and/or without renal injury. A great deal of investigation concerned with hyperhomocysteinemia associated with abnormal metabolism of methionine by enzyme and coenzym (vitamin B6 deficiency) actions is reviewed in this article. Moderate hyperhomocysteinemia is present in chronic renal failure, but its pathogenesis still remains unclear, although there are many hypotheses for the mechanisms of cell injury by homocysteine and for abnormal enzymatic methylation pathways,which convert homocysteine to methionine.It is known that heterozygote cystathione synthase deficiency among healthy persons may promote the progression of coronary arteriosclerosis from the mild hyperhomocysteinemia. Considering the high incidence of coronary and cerebrovascular accidents in dialysis patients,therapeutic intervention should be initiated to avoid risk factors such as hyperhomocysteinemia.
