[ Summary ] The basic points of view in therapeutic drug monitoring (TDM) of anti-epileptic drugs (AEDs) is reviewed. TDM is necessary to manage anti-epileptic actions, maintaining meaningful levels of blood concentrations of each drug. The optimal use of AEDs is under consideration, in that the selection of AEDs is based on the types of the seizures, the upper limits of concentrations for each AED are individual for each patient. Monotherapy is principally preferred. Considering the influences on liver and/or renal function, more interest must be paid in cases involving children and the elderly, or in the patients with liver and/or renal disease.
