[ Summary ] The significance of blood purification in treating ARDS (acute or adult respirtory distress syndrome)is briefly stated. ARDS should be considered to originate from SIRS (systemic inflammatory response syndrome)and/or to be a part of MOF (multiple organ failure). The pathophysiology is important because many causative factors are complicated, among which are humoral factors as well as neural factors and hormonal factors.Treatment of ARDS should be focused mainly on the removal of the basic disease and appropriate selection of the mode of respiratory care. However, there is a possibility of a use of blood purification in alleviating respiratory failure. Among several methods of blood purification, CHDF (continuous hemodiafiltration) seems to be superior because humoral mediators may be removed with minimal risk by using this mode, although other modes, such as endotoxin removal therapy, may be beneficial in certain situations.
In conclusion, further collection and comparison of precise data is necessary to determe the effectiveness of blood purification for the ARDS patients, although the therapy seems to be useful for some of them.
