[ Summary ] Acetate-free biofiltration (AFB or AFBF) is one of the better biocompatible hemodiafiltration (HDF) techniques which are performed with a base-free dialysate and simultaneous infusion in the postdilutional mode of sterile isotonic bicarbonate solution. The absence of acetate overloads during AFB might be the reason for asymptomatic dialysis treatment. Moreover AFB produces a better and easier correction of metabolic acidosis in patients on chronic hemodialysis. Other conventional HDF techniques using lactate buffer or bicarbonate with small amounts of acetate containing fluids,combining bicarbonate dialysate can not avoid the elevation of blood acetate levels. On the other hand, recently clinical trials of on-line hemofiltration (HF) employing the AK-100 ULTRA system (Gambro) with Bicart and JPN1 has documented. However, both AFB and on-line HF system have not been approved.
