[ Summary ] The pathological changes in destructive spondyloarthropathy (DSA) have various features, which include not only destructive intervertebral disc changes but also hypertrophic or granulomatous changes of the extradural spinal cavity including the posterior longitudinal ligament and the yellow ligament. Recently, a pseudotumor in an atlanto-axial lesion and destruction of the facet joint have been recognized in DSA.
Histological findings are characterized by infiltration of macrophages adjacent to deposition of HD amyloid, consisting of beta2-microglobulin. Multinucleated giant cells and osteoclasts are responsible for the destruction of bone and cartilage including intervertebral discs and vertebral body endplates. HD amyloid is also recognized in hypertrophic ligaments and granulation in extradural lesions.
