The structure and function of pulmonary vessels are extremely different from those of systemic vessels. Hypoxic pulmonary vasoconstriction, distension and recruitment regulate pulmonary circulatory hemodynamics.
Circulatory disturbance of the lung, i.e. pulmonary arterial hypertension, is mainly associated with the following four mechanisms : (1) Persistently increased pulmonary blood flow, (2) Elevation of left atrial pressure (Pulmonary venous hypertension), (3) Decreased pulmonary vasculature, (4) Hypoxic pulmonary vasoconstriction. Pulmonary hypertension is caused by one of these factors alone or a combination of two or more of the above mechanisms.
