[ Summary ] Posttransplant hypertension remains a common problem and has increased since the clinical introduction of cyclosporine. Causes of renal allograft loss include immunological and nonimmunological factors, and hypertension is one of the most important nonimmunological factors. Moreover, hypertension is an important risk factor for cardiovascular mortality in the transplant patient. Complicated mechanisms are responsible for posttransplant hypertension. Thus, optimal management of posttransplant hypertension relies on a careful analysis of its etiology and pathophysiology.
