[ Summary ] Chronic hemodialysis patients with low serum PTH levels have shown an increasing rate, recently. The serum PTH level is influenced mainly by the serum levels of Ca, P and vitamin D and has a profoundly significant relation to adynamic bone. Evaluation of parathyroid function only by means of the serum PTH level is not consistently accurate. Therefore,Ca-free hemodialysis was carried out in 24 patients. Eleven out of 24 cases (45.8%) were unresponsive, that is, in these cases the peak intact PTH was less than 200pg/ml despite acute hypocalcemic stimulation (serum intactPTH>200 pg/ml is tentatively considered to be the level needed to maintain normal bone turnover). These results indicate that Ca-free hemodialysis is effective and valuable for evaluating parathyroid function.
