[ Summary ] We examined the usefulness of endoscopic ultrasonography (EUS) for large sessile colorectal epithelial neoplasms. EUS was attempted in 48 lesions with colorectal neoplasms over 20mm in diameter. Evaluation by EUS was possible in 79.2% (38/48). The diagnostic accuracy rate of EUS was 81.6% (31/38) for those tumors over 20mm and 88.5% (23/26) for tumors under 19mm in diameter. The diagnostic accuracy rate of EUS was 73.9% (17/23) in protruding lesions and 90.2% (37/41) in flat elevated lesions. In protruding lesions, especially over 20mm, the diagnostic accuracy rate was 55.6% (5/9). It is difficult to diagnose the depth of invasion because most of these lesions are over 11mm in height. Marked attenuation of ultrasound was noted in these cases. On the other hand, EUS is useful for the evaluation and diagnosis of tumor invasion in flat elevated lesions because it provides a high accuracy rate.
