[ Summary ] We investigated the usefulness and limitations of endoscopic ultrasonography (EUS) for determining the indications for endoscopic mucosal resection (EMR) for early colorectal carcinoma. EUS was performed on 58 lesions with early colorectal carcinoma. 53 lesions (91.4%) were detectable by using EUS.
EMR can indicate the presence of carcinoma, which has minimally invaded the submucosa. In this study, submucosal carcinoma was divided into three groups, sm1, sm2, and sm3 according to the degree of submucosal invasion. The diagnostic accuracy rate of EUS was 83.0% (44/53) in m-sm1 carcinoma and 81.3% (13/16) in sm2-3 carcinoma. The diagnostic accuracy rate was 75.0%, 86.4%, and 90.9% for elevated types, flat elevated types and depressed types, of tumors respectively. For those tumors over 20mm in diameter, the accuracy rate for the elevated type tumor was lowest (50%). In the elevated type tumor, it is difficult to diagnose the depth of invasion due to histological and morphological factors.
However, it is concluded that EUS is useful in determining indications of early colorectal carcinoma.
