Title Usefulness and pitfalls of magnifying colonoscopy --comparison between indigo carmine dye and crystal violet staining

[ Summary ] Chromoendoscopy is useful in observing the tumor margin and surface. Moreover, mucosal pit patterns can be assessed when a magnifying endoscope is used, making it possible to estimate the histology of the tumor. It is generally accepted that small rounded pit patterns (type IIIs pit) and destroyed pit patterns (type V pit) can not be clearly seen when only using indigo carmine staining, and for that purpose crystal violet staining should be applied. With crystal violet staining we can get beautiful pictures, but it takes a few minutes and is somewhat cumbersome. In this paper, we evaluated three studies comparing indigo carmine and crystal violet staining. We evaluated these factors : 1) the correlation between mucosal pit patterns, 2) the correlation of macroscopic types of small colorectal tumors, 3) the detectability of type V crypt patterns in early colorectal carcinomas. For the best results, crystal violet staining was superior to indigo carmine staining in every studies. But crystal violet staining can not be used to examine all lesions. This type of observation is necessary for the recognition pitted of tumors, such as those suspected to have the depth of invasion common to early colorectal carcinomas.
