[ Summary ] The 14,716 patients studied were referred to our clinic for a colonoscopy between November 2007 and July 2021 and comprised the study population. Among them, 8618 lesions were histopathologically diagnosed. Fifty IIc lesions were extracted from the endoscopy filing system. Approximately 50 % of the IIc lesions were located in the rectosigmoid colon. The rates of early colon cancer and submucosal invading cancer were 42 % and 30 %, respectively. The most important point is that submucosal invasion of IIc cancer usually occurs 5-10 mm in diameter and 10-15 mm deep, invading the deep submucosal layer. The IIc type of depressed lesions is an important route in the development of colorectal cancer. This remains a universal concept, even 30 years later.
