[ Summary ] In 1977, a colorectal 0-IIc lesion, which was previously regarded as a "phantom cancer," was detected during the increased occurrences of early cancer diagnosis by radiography and endoscopy. Subsequently, the authors detected many colorectal 0-IIc lesions. They suggested that development of depressed lesions was the main route towards colorectal cancer development, and considered the lesions as de novo cancer. By publishing "Early Colorectal Cancer" and launching "Annual Meeting on Depressed Colorectal Cancer," we have been trying to spread the importance of 0-IIc colorectal lesions. Here, we look back on the history of 0-IIc, including the latest developments, such as artifi cial intelligence and gene analysis, which the authors are mainly working on.
