[ Summary ] Although a reduction in the incidence and mortality of colorectal cancer (CRC) with flexible sigmoidoscopy screening has been shown in randomized controlled trials (RCTs), the effectiveness of total colonoscopy (TCS) remains unknown. The Akita Japan Population-based Colonoscopy Screening Trial (Akita pop-colon trial) is a RCT that investigated the effectiveness of using one-shot TCS combined with the fecal immunochemical test (FIT) as a screening program. The primary endpoint was CRC mortality, and the secondary endpoints were cumulative CRC incidence, CRC screening sensitivities and specificities, and prevalence of adverse events. Recruitment of participants was terminated by the end of March 2017. In total, 9732 people agreed to participate in this study and were randomly allocated to the FIT (n=4871) and TCS (n=4861) groups. TCS screening was performed in 92 % of the participants, with a success rate of 99.7 % in cecal intubation. No severe adverse events were reported. This RCT could reveal the effectiveness of TCS screening for CRC mortality.
