[ Summary ] A man in his forties with non-profuse type familial adenomatous polyposis (FAP) underwent surveillance colonoscopy because of his refusal to undergo prophylactic total colectomy. Colonoscopy revealed a depressed-type lesion measuring 6 mm in the transverse colon. After endoscopic mucosal resection (EMR) and subsequent histopathological analysis, the tumor was designated as a moderate-to-poorly differentiated adenocarcinoma, 5x5 mm, T1b (SM 3200μm), LyX, V1, HM0, VM±. Additionally, total colectomy revealed no residual tumor at the EMR scar, and no lymph node metastasis (pT1bN0M0) was observed. Furthermore, approximately 1000 colon polyps were confirmed in the entire colon. Thus, when cases with colonoscopy findings without total colectomy are investigated, phenotypes, genotypes, close surveillance examination intervals for early detection of cancer, and the risk of depressed lesions must be carefully considered before making a decision. Herein, we report the case of a small depressed-type colonic SM cancer that showed cancerous growth pattern in a FAP patient.
