[ Summary ] |
We present a case of a descending colonic lesion detected during a previous colonoscopy for a positive fecal blood test. Magnified endoscopy with narrow-band imaging and crystal violet staining suggested that it was a deeply submucosal invasive cancer despite its small size of 8 mm. Laparoscopic colectomy with lymph node dissection was performed as the initial treatment. Pathology revealed lymph node metastasis; thus, he was diagnosed with colon cancer, stage IIIA. Depressed morphology and non-polypoid growth suggested that this lesion progressed via the "de novo" pathway. As "de novo" cancer is considered to deeply invade the submucosa even when it is small in size, small non-pedunculated T1 cancers may potentially include many "de novo" cancers. Currently, their clinical, pathological, and molecular characteristics have not been fully evaluated. |
