[ Summary ] Most depressed-type colorectal cancers are considered to be de novo cancers, developing from the de novo pathway. Compared with those arising from the adenoma-carcinoma sequence, depressed-type cancers from the de novo pathway (de novo-type cancer) are characterized by aggressive submucosal (SM) invasion and a high occurrence of lymph node metastasis, even if the lesion size is small. Herein, we reviewed the previous literature and our database of T1 cancers to analyze the risk of SM invasion, metastasis, and recurrence of de novo-type cancer. Although there is no consensus on the definition of de novo-type cancer, the results show that de novo-type cancer is more biologically malignant than adenoma-carcinoma sequence-derived cancer. Future research with high-level evidence is expected to reveal the precise biological features of de novo-type cancers, which can have a great influence on the treatment choice for patients with colorectal cancers.
