[ Summary ] The characteristics of fecal immunochemical test (FIT)-negative colorectal cancers (CRCs) were investigated based on the results of a populationbased screening trial using two-sample-FIT and total colonoscopy performed in Oshima, Tokyo. All FIT negative CRCs were intramucosal cancers and were completely treated by endoscopic resections. FIT-negative CRCs had the trend of early stage, right-sided, and flat type compared to the FIT positive CRCs. Development of new, effective, and organized screening methods combined FIT and colonoscopy are desired.
