[ Summary ] We retrospectively compared the clinical histopathologic characteristics of colorectal cancers that tested as positive and false negative on a fecal immunochemical test (FIT). Early-stage cancers that test as false negative on a FIT are often small in diameter and shallow in depth of invasion; superficial tumors are also likely to be false negative. Advanced cancers that test as false negative are more common in women, and are more likely to develop in the right side of the colon (especially the cecum). As a countermeasure to identify false-negative results, it is important to perform sufficient quality control while understanding the features of FIT false-negative cancers. In addition, colonoscopy and CT colonography are considered countermeasures to compensate for this weakness of FIT.
