[ Summary ] Endoscopic submucosal dissection (ESD) for early colorectal tumors has been covered by Health Insurance in Japan since April 2012 on the basis of its effectiveness and safety. As techniques improve, even for lesions for which we avoided endoscopic treatment, and instead chose operations, ESD has come to be performed frequently. In order to provide safe and reliable treatment, it is important to respond promptly when adverse events such as perforations occur. It is essential to recognize the characteristics of OTSC and perform procedures such as closing methods to guard against perforations or damage to muscule layers, which are difficult to close with clips.
