[ Summary ] We reported on the first case of collagenous sprue in Japan and showed the progress being made in improving mucosal appearance with repeated capsule endoscopy. Collagenous sprue causes severe malabsorption with villous atrophy and subepithelial collagen bands in the proximal small intestine. This disease is obviously different from celiac disease in terms of serological features (negative for anti-IgA tissue transglutaminase or HLA-DQ2) as well as poor response to low-gluten diets. Recently, collagenous sprue has been suggested to be possibly related with IgG4-positive plasma cells or medications including ARBs and NSAIDs. We review recent evidence concerning collagenous sprue in reference to our experience.
