[ Summary ] A woman in her 70s underwent an initial colonoscopy for FOBT(+) and abdominal distension. Resection of a polyp 8 mm in size in the sigmoid colon was conducted. She underwent subsequent colonoscopy 12 month post initial colonoscopy for hematochezia at which time advanced rectal cancer (Borrmann type 2) was found in the rectosigmoid junction. Interval advanced cancer was thought to have been hidden during the initial colonoscopy by a strongly bending portion along with a blind area in the rectosigmoid junction. Endoscopists must investigate blind areas in detail by not only pulling but pushing the colonoscope and attaching transparent hoods to the head of the colonoscope.
