[ Summary ] Endoscopic polypectomy with electrocautery for small colorectal neoplasms is still considered as a standard procedure. However, in recent history, polypectomy has included hot biopsy/hot snare polypectomy (hot polypectomy : HP) and cold polypectomy (CP).
An advantage of CP is its low incidence of adverse events such as delayed perforation. In contrast, HP is a safe procedure if a tumor is pulled sufficiently into the internal colon lumen in order to avoid delayed perforation related to excess burning effect to the muscle layer. As the incidence of adverse events is very low, CP will expand in Japan as it has in western countries. A disadvantage of CP is the possibility of residual/recurrent tumors.
It is necessary to perform image-enhanced endoscopy with magnifi cation before and after CP endoscopic resection in order to confirm removal.
