[ Summary ] The term "Laterally spreading tumor(LST)" was proposed by Kudo( 1991), and LSTs were classified into two types, granular and non granular. We have reported that the granular type is different from the non-granular type in terms of macroscopic features, location, crypt pattem, histological findings, genetic background (p53 overexpression, and K-ras point mutation).
Based on these results, it seems reasonable to suppose that the granular type is a lesion, based on a adenoma-carcinoma sequence, and the non-granular type is simailar to so called "de novo" carcinoma. In this study, each type of LST of the granular and non-granular type was compared with other macroscopic types of early colorectal cancer (protruding type I , superficial elevated type IIa and IIb, depressed type of IIa+IIc and IIc) according to the clinicopathological features. In clinicopathological and genetic findings, the granular type of LST is similar to the protruding lesion of type I and the superficial elevated type IIa, non-granular type LSTS are similar to the depressed type of IIa+IIc and IIc.
