[ Summary ] Here we discuss the diagnosis and treatment of primary gastrointestinal follicular lymphoma (GIFL), based on the reports and clinical features of GI-FL patients treated and observed in our hospital between 2001 and 2013. We evaluated 31 cases of GI-FL. Typical endoscopic features of duodenal involvement were multiple, whitish, small polyps and whitish granules. Double-balloon endoscopy and/or capsule endoscopy revealed that most patients with GI-FL had multiple lesions, spanning from the duodenum to the ileum. Infiltration of FL cells could be detected in the bone marrow. Because accurate diagnosis and staging are needed for the management of this disease, it is essential to examine the entire small bowel and the bone marrow. There is no established therapy for patients with GI-FL. Further analyses with larger numbers of FL patients are needed to clarify the best therapeutic approach for GI-FL.
