[ Summary ] Endoscopic treatment strategies in consideration of long term prognoses for submucosal colorectal cancer were examined. The clinicopathological findings concerning colon cancer deaths occurring after local excision and colectomies were reviewed. In addition, deaths due to factors excluding colon cancer patients 65 years old or older experienced more than 3 times the colorectal cancer-related deaths. Treatment strategies in consideration of long term prognoses for submucosal colorectal cancer can include removal with an endoscope as a first line treatment. Clinicopathological findings from local excision lead us to consider whether additional colectomies are required. However, submucosal colorectal cancer in elderly patients may require follow ups, even if clinicopathological findings indicate additional colectomies.
