[ Summary ] The risk of thromboembolism may be balanced by that of hemorrhagic complications during medical procedures in patients receiving antithrombotic agents. Thus, management of anticoagulants or antiplatelets has become an important issue. Brain infarction occurs in approximately 1 % of patients when anticoagulants are interrupted, often with severly poor outcomes. Guidelines from the Japan Gastroenterological Endoscopy Society have been greatly revised. The goal has changed from avoiding hemorrhaging to avoiding both hemorrhaging and thromboembolism. From the standpoint of stroke neurologists, protecting patients from thromboembolisms associated with discontinuation of antithrombotic agents, the newly revised guidelines may achieve improvements in managing antithromboltic agents during endoscopic procedures.
