[ Summary ] Antithrombotic agents (antiplatelet and anticoagulant agents) are widely used for patients with cardiovascular disease. Anticoagulants may be used for atrial fibrillation, prosthetic valves, venous thrombosis and antiplatelet therapy for arteriosclerotic disease including angina, myocardial infarction or peripheral artery disease. Taking antithrombotic agents is associated with an increased risk of bleeding, especially gastrointestinal bleeding. However, discontinuation of antithrombotic agents is related to increased risk of thromboembolic complications. There is little evidence of particular safety zones for treatment or duration of discontinuation when noncardiac surgery or procedures are needed during antithrombotic therapy. We would like to emphasize the need for cooperation between gastrointestinal and cardiovascular physicians in the management of patients taking antithrombotic agents.
