[ Summary ] Cytomegarovirus infections (CMV) are associated with refractory ulcerative colitis. It should be noted that the term "CMV reactivation" is different from the term "CMV colitis". CMV infections are diagnosed primarily through use of antigenemia assays whereas diagnosis of CMV colitis must be detected through observation of CMV in the colonic mucosa. This is accomplished by employing histology, immunohistochemistry, and PCR amplification assays of the colonic mucosa. Previously our studies have demonstrated that patients who are CMV antigenemia positive and are refractory to steroids or immunosuppressants do not require antiviral treatment. Recent studies have indicated that PCR amplification assays in the colonic mucosa are useful in diagnosis of CMV colitis and determining refractory UC. However, large prospective studies are necessary to ascertain new treatment options.
