[ Summary ] Colorectal endoscopic submucosal dissection (ESD) requires a high level of skill and experience in therapeutic endoscopy because of the high risk of complications such as perforation and bleeding. Therefore, a greater understanding of the learning curve for ESD is required to standardize training and achieve wider acceptance of this technique.
At our institution, trainees must meet the following prerequisites to perform colorectal ESD:a high level of skill in the non-loop insertion colonoscopy technique (more than 10 cases of total colonoscopy completed within 5 minutes each without any abdominal discomfort), experience with > 20 gastric ESDs and assisting during > 20 colorectal ESDs conducted by experienced endoscopists. Skill in conventional EMR or piecemeal EMR techniques is also needed prior to performing colorectal ESD.
Based on short-term outcomes for colorectal ESDs performed by trainees in our institution, colorectal ESD can be performed without serious complications by trainee endoscopists under the guidance of experienced specialists. We believe that trainees require an experience level of > 30 cases to perform colorectal ESD without guidance from an experienced specialist.
