[ Summary ] Endoscopic submucosal dissection (ESD) is the preferred procedure for en bloc endoscopic resection, even for lesions with severe submucosal fibrosis. However, this procedure requires a high level of skill and experience for operators due to technical difficulties and a high risk of perforation. Difficulties with lesions must be considered before starting procedures to provide safe, successful ESD. This kind of difficult colorectal ESD procedure should be conducted only by experts.
