[ Summary ] A 50-year-old man was hospitalized for treatment of a rectosigmoid (RS) tumor after being referred from an affiliated hospital. Standard colonoscopy exhibited a reddish superficial-elevated tumor with a pseudo-depression (IIa + IIc type) 20 mm in diameter. There was a small elevation in the depressed area. Narrow band imaging with magnification revealed an irregular surface and vascular patterns. A small elevation in the depressed area revealed the absence of a surface pattern and an irregular vascular pattern was observed. Magnifying colonoscopy with crystal violet staining of a small elevation exhibited type VI pit pattern. These findings indicated that this tumor displayed elevated submucosal invasion in a depressed area. Endoscopic submucosal dissection was performed. Histologic findings revealed well differentiated adenocarcinoma. Shallow submucosal invasion was observed with small elevation in at least three areas. Curative resection was carried out. No vessel involvement was observed.
