[ Summary ] We investigated 277 small colorectal cancers under 2.0 cm in diameter to estimate their clinicopathological characteristics. Coexistence with adenomatous components is high, with a rate of 61.7 %. The histological characteristics indicate a low rate of coexistence with adenomas, a high percentage of cancer invasion, components exhibiting moderate and poor differentiation. A high percentage of sprouting tend to be present in groups with larger lesions, submucosal or deeper invasion, and lymph node metastases. Lesions associated with lymph node metastases tend to express CD10 and may be supported by high-grade malignancy related with small intestinal-phenotypes. As a result, it is important that we diagnoses small colorectal adenomas precisely.
