[ Summary ] In accordance with Japanese Society for Cancer of the Colon and Rectum (JSCCR) guidelines 2010 for the treatment of colorectal cancer, the results were as follows;when invasive SM colorectal carcinomas were divided into pSM1 (measuring the level of SM invasion, <1,000μm) and pSM2 (≥ 1,000μm), lymph node metastasis was found in more than 30 % of pSM2 with budding/sprouting (grade 2-3). Tanaka S and Ajioka Y et al. demonstrated the appropriateness of the above on the basis of surveillance results after endoscopic treatment for invasive SM colorectal carcinoma. On the other hand, Matsuda T et al. particularly emphasized the specificity of Ip-type SM colorectal cancer in their articles. They concluded that because Ip-type cancer was more easily treated with endoscopy than non-Ip-type cancer, and because it had lower rates of recurrence and metastasis, it is clinically important to differentiate both with endoscopy. Careful reading and understanding of the individual references and guidelines is recommended to understand details.
