[ Summary ] We analyzed the endoscopic differential diagnosis of depressed (or flat) type neoplastic lesions and non-neoplastic lesions of the colo-rectum, and summarized the following three important points among several findings : (1) Most depressed neoplastic lesions showed slight erythema, while non-neoplastic lesions such as mucosal erythema, endoscopic trauma and vascular ectasia presented marked erythema. (2) Innominate grooves were absent from the neoplastic lesions on dye spraying, most non-neoplastic lesions, however, had innominate grooves. (3) When air volume was increased, marginal elevations surrounding the neoplastic depressions were higher and neoplastic depressions appeared deeper. In non-neoplastic lesions, shape deformity with air volume was not recognized. It is very important for the detection of neoplastic depressed lesions that colonoscopists understand the combination of above the important findings.
