[ Summary ] Owing to the aging population in Japan, we have to perform gastrointestinal endoscopy in the patients with high thromboembolic risks. Antithrombotics are divided into antiplatelets, anticoagulants, and thrombolytics. The previous guidelines regarding gastrointestinal endoscopy in patients with antithrombotics recommended stopping the agents, but the newest guidelines allow practitioners to perform gastrointestinal endoscopy without cessation of the agents. The evidence levels of the statements, however, are quite low, and further active clinical research regarding this topic should be actively conducted in future.
