[ Summary ] In patients with Lynch syndrome, there are few cases in which total colectomy is indicated when colorectal cancer is diagnosed, therefore gastroenterologists are responsible for the surveillance of metachronous colorectal cancer in the remnant colon after surgery. Additionally, gastroenterologists should have responsibility for the surveillance of not only the gastric and duodenal cancer among lesions other than the large intestine but also those of at‒risk relatives. Japan leads the world in rates of endoscopic diagnosis and treatment of gastrointestinal cancer. However, to ensure accurate endoscopic diagnosis and early detection of lesions in patients with Lynch syndrome, gastroenterologists should be actively involved in the medical treatment of patients with Lynch syndrome and be aware of the cancer risk in this group. This article discusses recent multicenter clinical research reports from western countries in addition to an outline of the risk of gastrointestinal cancer in patients with Lynch syndrome.
